[ASTERIXDB-2284][CLUS] Ensure Node Failure on Heartbeat Miss

- interface changes: no

Details:
- Request the node which exceeded its heartbeat misses
  to shutdown to ensure its failures.
- Ensure thread safety of lastHeartbeatNanoTime in
  NodeControllerState.
